for Completing a Divorce Certificate Request
For
a copy of a divorce certificate, fill out the
application
form on this Web server or send a letter including the husband's first
and last name, the wife's first and maiden name, the date of divorce, the
place of divorce, the reason you need the record, and your relationship to
the person whose record is requested. The fee is $15.00 per copy. Checks
or money orders should be made payable to the New York State Department of
Health. DO NOT SEND CASH.
